4. How to Request Access

To request access to records held by the Practice, submit a written request to the Information Officer using the prescribed Form C (available from the South African Human Rights Commission website). Include:

• Your full name and contact details
• A description of the record you wish to access
• The form in which you require access
• Proof of identity

5. Processing Fees

A request fee is payable as prescribed by the Regulations to PAIA. Medical records requested by the patient themselves are ordinarily provided at cost of reproduction only. Fee waivers apply for personal medical records.

6. Grounds for Refusal

Access may be refused on grounds including protection of third-party privacy, commercial confidentiality, and where disclosure would be contrary to public interest or legal professional privilege.

7. South African Human Rights Commission

If your request for access is refused or not responded to within the statutory period, you may approach the South African Human Rights Commission or a court for assistance.
